Many of the positions listed on our website are temporary or temp-to-perm positions. Temping is a great way to enter any field and it allows both you and our client employers to make sure the position is a great fit for you. We also offer contract positions for many professional roles, including Physical Therapists.

This section of our site is designed to provide you with forms and documents necessary to place you with our clients.

It is important in any healthcare position where you will be in contact with patients and other medical staff that you have accurate and up-to-date immunization records. Please refer to the following document Immunization Record Instructions. You will need to scan your records and e-mail them to RFarina@signaturestaffing.info.

If you are seeking a position in IT/Software Engineering, please open and fill out the Software Questionnaire in Excel and send it to: RFarina@signaturestaffing.info with both your name and “Software Questionnaire” in the subject line. You will be contacted soon with appropriate job matches or we will retain your information for future job openings.

In addition to the Application, we ask that temps/contract workers fill out employment paperwork to ensure that when we call, you are ready to work right away. You will be asked to fill these out when you come in for an office interview, but the forms and instructions are provided below for the convenience of phone interview candidates.

The following are instructions for the five forms required to temp:

  1. Fill out the first two top sections of the New Employee form
  2. Fill out items 1 through 7 on the bottom of the W-4 and sign
  3. Fill out Section 1 of the Form I-9
  4. Sign the Confidentiality Agreement

Once you are assigned a temp position, you will need to fill out at Time Sheet every week. That document is also provided below. Time Sheets must be faxed by 5:30 pm each Friday to be considered for payroll.
